Covered operations need preapproval inspection, a 24-hour emergency phone, accessible records, and a written follow-up agreement with a fixed facility within 25 miles. The Board may waive that distance when no facility is available. File sites and addresses with the application and update as necessary every 30 days.
Anesthesia is limited to local anesthetics. Administrative companies cannot own the practice. Review the rule’s consent, patient-information, equipment, infection-control, personnel-change, and closure duties.
