Abstract

A secondary analysis of semi-structured interviews with 16 participants from 10 pharmacist-driven MHCs was conducted. Barriers and facilitators were mapped to CFIR constructs. Top matched strategies to address barriers: identify and prepare champions; assess readiness; promote adaptability; capture and share local knowledge; conduct local consensus discussions. 11 strategies were selected for model incorporation.
