Abstract

This report describes a human resources approach to training and deploying community health workers (CHWs) within mobile health clinics. Partnering with Family Van (Harvard-affiliated), CHWs completed training in motivational interviewing, health coaching, and social needs screening. The model achieved high staff retention and satisfaction. CHWs conducted 3,200+ encounters annually, with 87% of patients reporting improved understanding of their health.
