Navigating the Terrain of Transitional Care and Mobile Health Services for Stroke Survivors: A Scoping Review.

Abstract
This scoping review examined transitional care and mobile health services for stroke survivors. Twenty-two articles were included. Interventions included home visits, telehealth, and mobile health apps. Key outcomes evaluated included functional recovery, medication adherence, and quality of life. Community-based transitional care improved patient outcomes when combined with mobile health technology and multidisciplinary support.
